2) Bring a picture ID, vision and medical insurance cards, and a list of all medications and allergies.
3) TO PURCHASE GLASSES FROM OUR BREMERTON LOCATION, PLEASE CALL TO SCHEDULE AN APPOINTMENT.
4) Minors or adults with special needs may be accompanied by a guardian or caregiver. Due to space restrictions and ongoing COVID guidelines, other visitors may be admitted on a limited basis.
5) Please cancel your appointment if your are exhibiting signs of illness including fever and coughing or if you have been exposed to someone that has tested positive for COVID-19.
